The BFG CAF provides the means for practitioners and organisations to work together to support individual children and young people meet each of the 5 Outcomes.

Supporting Forms

The following forms are accepted by all agencies/organisations in BFG.  They should be used as practitioners feel appropriate and useful.  No additional paperwork would normally be required.  Boxes will expand as you type into them.

The forms are protected; this results in the spell-check not being operational and the footer being unavailable.  To unprotect the form once it is completed (to spell check, and add name to footers), go through the view menu, select toolbars, forms.  When the new toolbar appears, click the picture of the padlock, once completed re-click this to lock the form. 

Checklist

Complete this checklist to help you decide if a more detailed assessment and/or the support of other professionals maybe useful to support the unborn child/child/young person (further referred to simply as the child).

Personal

Holds all relevant personal details of the child (such as address, parent details etc) can be completed once and fixed to any other section as required.

Details

Assessment

Use the assessment to document the discussion you have with a child/young person and family to identify the strengths and needs.



Action
Support Plan & Multi Agency Meeting Agenda/Minutes - this form to be used to co-ordinate support of more than one service/agency/practitioner. It can be used to record multi agency meetings, when it will also act as a prompt for the agenda.

Request

Use this (with the assessment or a checklist in certain circumstances)  to request the support of any other agency/organisation or to invite another professional to a multi agency meeting.


Meeting

Used as an agenda/minutes for a multi-agency meeting (can be completed by hand and photocopied at the meeting), can also be used to document multi agency actions (as an alternative or in addition to Action plan)

Additional

Used and added to other documents if anything changes, or more detail is required.



Closure

This form should be used at the end of a CAF intervention.  This may be when the support has led to the needs being met, the child/young person returns to support from purely universal services, needs cannot be met or referral is made to BFSWS, or the child undergoes a transition (ie posted, moves to another LP or setting).
